in the center of the hospital, the emergency room has public entrance through the front doors, off the main lobby, but also an ambulatory entrance through a direct access door. All patients and ERT Members will be brought here for treatment

Patient cyanotic. Last dosage of albuterol with a nebulizer was that morning. About to play basketball when asthma attack ensued. No known allergies. Started using O2. After a round of O2, the O2 with nebulzier was used. Skin color turned less pale. Took vitals. Administered albuterol with nebulizer. Used nasal cannula as supplemental. Patient able to walk with assistance to ambulance. Transferred patient to hospital.

Upon request SCFD Lieutenant assisted in removal of turnout coat and SCBA pack. Once removed BLS took VS and exam. Exam findings are slightly slurred speech uneven pupils. VS were noted to be slightly elevated. PT ambulated to stretcher upon request. O2 sat was 98. ALS started a saline lock for access and started 02 at 4lpm via NC for PT comfort. PT transported to SCH. Upon arrival at hospital PT care transferred to ER staff

M-1 Dispatched to MVA. Upon arrival 65 yo male PT found unresponsive slumped over steering column of rear vehicle involved in rear end MVA PT noted to be not breathing. FD broke out window for preliminary PT access.Carotid pulse and breathing assessed via window access. PT -abcs Unresponsive. FD popped  passenger front door and assisted in extrication. BLS performed CPR while ALS started 12 lead EKG.PT noted to be astoyle. Due to extended extrication Medical control contacted to request discontinuation of resuscitation efforts. Medical control pronounced at 0657 and CPR discontinued. ME notified PT transferred to the ME and PD for identification and notification.

*Pulling up to the Hospital's bay I shut off the engine, open the door, and head around to the back of the truck.  Opening the back door I help unload the stretcher with the patient on it.  I assume Breslin has the PCR so I just wheel the stretcher into the ER.*

*As the ambulance pulls into the bay and stops I get out of the back while the EMS crew unloads the suspect/patient.  The teen girl named Nicole is sleeping and that's fine; I can question her later since I know who she is and where she's at.  I follow them into the ER real quick and then leave as I go back out to the bay to wait for Sgt. Jones to get a ride back.*

*As we roll into the hospital, I fell us come to a stop and I watch as the deputy opens the back doors. Alyssa and I get the stretcher out before we extend the legs and start into the ER. We hand her off to one of the doctors and I head to the nurse’s station to fill out the PCR.*

Medic 1, consisting of Paramedic Alyssa Breslin, EMT Aurora Floyd, and Paramedic Cadet Sarah Brooke arrived on the scene of a disturbance. Upon patient contact, a Sheriff’s deputy from Shiloh County Sheriff Department was holding down a female patient. Paramedic Breslin administered 5 mgs of Haldol before she instructed EMT Floyd and Paramedic Cadet Brooke to get the patient to the ambulance. The patient was brought to the ambulance where Paramedic Cadet Brooke took over the role of primary caregiver, supervised by Paramedic Breslin, who instructed Cadet Brooke to get a set of vitals, begin an IV, and to get the patient’s medical history. Cadet Brooke took a set of vitals which were a blood pressure of 135/80, Pulse of 55, and Reps of 10. This information was gathered from the EKG, which provided a strip showing the the heartrate was a minor Sinus rhythm. The patient then advised Cadet Brooke that she was not on any medication, nor was she allergic to any medication. She also provided contact information for her parents though her personal cell phone, which was examined by SCSD Deputy Evelyn Hawke, who rode with the Medic 1 crew. Finally, Cadet Brooke began an IV into the patient’s left arm and began a Saline drip. The Medic 1 crew informed the hospital of the patient’s condition before they arrived and handed the patient off.

*Brushing off snow from 100's door handle so I can see it, I open the door and get into the SUV's passenger's seat.  Shutting the door quickly to keep as much snow out of the cabin as possible I buckle up and look to Jones to let him know I'm ready to go.*

M2 was dispatched to the intersection of Route 40 and Broad for an MVA involving a FD unit.  Upon arrival we immediately went to work on the driver of a vehicle that went into a ditch.  Vitals were taken and for safety reasons, because the driver was inebriated and speaking incoherently, we used the C-Spine and KED.  With help of firefighters we then extracted the driver from the vehicle by using the scoop to bring him to the stretcher.   We then loaded the patient into the back of M2 and transported him to Shiloh County Hospital.

*Killing the lights and sirens I pull into the hospital's loading area where I see Breslin waiting for us by the door as I shut off the engine.   Opening the door I jump out as I rush to the back of the truck and help open the back doors for the others.   Getting the stretcher out of the truck I help the others wheel it inside until suddenly my queasiness gets worse.  Breaking off from the group being thankful I don't have the PCR, I head to the bathroom, get inside a stall, and promptly vomit out my lunch from earlier into the toilet.*

*I nod as I get the information.  I want to keep him talking if I can and try to keep his mind off of the pain and to keep him calm.  I know that we’re getting close to the hospital, so we can turn him over soon to the hospital.  When I hear from dispatch they want us out of service, I raise an eyebrow.  We’re in the middle of transporting a patient so it’s not like we can just return to quarters.  I let Erika take it since Duncan and I are working on the patient in the back and I want to keep his attention.  The last thing that we need is to freak out about his injuries and possibly put himself into shock.  As we pull up at the hospital, I wait for the trauma team to open the back door.  I let Duncan get the IV bag off or our hook as I start filling in the doctors. Just before I head in to do the PCR, I see Breslin standing by the side of her car. As she walks toward us, I figure that it's directed for us, so I head over.*

 

'Lieutenant. What's up?'

'Yeah I heard that they wanted us out of service, but I mean we were in the middle of a transport, so I didn't hear any details if there were any.  I don't know why they'd want us out of service.  If anything, I'd assume that they want the cop's gun from us.  I got one of the cops to get it out of his hands and we put it in the lockbox.  I've never transported a cop before, so I thought that was what I needed to do.'

*After cleaning myself up a bit I leave the bathroom and exit the hospital as I head back to M1.  Spotting Breslin with Brooke at the truck I let my hair down from the ponytail it was in as I finish the trek over.*
